Tara Reid says she is going to pursue legal action after distressing footage showed her being taken out of a Chicago-area hotel on a stretcher.

In a police report filed with the Rosemont Police Department and obtained by PEOPLE on Tuesday, Nov. 25, the actress, 50, said she “completely blacked out” after having a drink with a stranger while staying at the DoubleTree Hotel in Rosemont, Ill. for a celebrity signing on Sunday, Nov. 23.

The American Pie actress recalled there being “a bunch of people in the lobby that were supposedly YouTubers.” She added that most had “their phones out and the whole thing felt suspicious but I didn’t know what was going on.”

In the police report, Reid said she ordered a glass of white wine from the hotel bar and then “A man came up to me and introduced himself as Sean P, a very successful influencer and YouTuber,” and joined her at the bar.

They ended up going outside to have a cigarette and while talking, exchanged phone numbers.

When she came back inside, “there was a napkin over my drink,” Reid alleges in her police report.

“Then the last thing I remember was drinking that drink and seeing a couple sitting next to us. The next thing I remember was waking up in the hospital around 8 hours or more later not knowing where I was or anything that happened. I completely blacked out with no recollection of anything,” she told police.

Authorities arrived at the scene at approximately 12:39 a.m.

Video obtained by TMZ showed Reid seemingly unable to stand on her own as a man held her up.  Two others stood nearby, with a woman bringing a wheelchair for the Josie and the Pussycats actress.

“Give her a second,” a man is heard saying as several individuals are gathered around Reid. One of the men forcefully pulls on her coat to place her in the wheelchair, with the initial man who was holding her up extending his free arm to tell the other person to stop.

“That is her,” a woman says off-camera, recognizing Reid, as they watch the situation unfold in the two-minute clip.

Once seated in the wheelchair, the actress was provided bottled water, but spoke incoherently as she was wheeled away. Before she was placed on the stretcher, video showed her slouching, almost completely out of the wheelchair.

Reid said her agent found what hospital she was taken to, and picked her up. They went back to the hotel so she could collect her belongings and then she returned to Los Angeles.

“I have been trying to file a police report since I got back to LA and they said that I could not file one over the phone and had to be there in person,” the actress said, adding that she was eventually allowed to file the report without having to return to Illinois.

Reid alleged that the influencer she met “texted me the videos of me the next day,” which she considered to be an extortion attempt as he allegedly told her he “could stop the videos [from] going out.”

She added that he confirmed Reid only had one drink while they were together.
